Question 1

Can WOMAC be used for other joints (e.g., hands, spine)?

Accepted Answer

WOMAC was developed and validated specifically for hip and knee OA. For hand OA, the AUSCAN index is the analogue. For spine, the Roland-Morris Disability Questionnaire is preferred.

Question 2

How does WOMAC compare to the HOOS/KOOS?

Accepted Answer

HOOS and KOOS extend WOMAC by adding sport/recreation and quality-of-life subscales. They include the full WOMAC as their pain/stiffness/function components.

Question 3

How should I interpret changes in WOMAC scores longitudinally?

Accepted Answer

A worsening of ≥4 points on pain or ≥7 points on function (Likert) is considered meaningful deterioration. The OMERACT-OARSI responder criteria combine pain, function, and patient global.

Question 4

Is WOMAC affected by comorbidities?

Accepted Answer

Yes — WOMAC function and pain items can be influenced by other musculoskeletal conditions affecting the lower limbs. Consider using patient global as a complementary measure.
