Question 1

Can minimal HE (grade 0) be diagnosed without psychometric testing?

Accepted Answer

Minimal HE cannot be reliably diagnosed at the bedside. Dedicated psychometric tests such as the Psychometric Hepatic Encephalopathy Score (PHES) or the EncephalApp Stroop test are required. It is clinically important because it affects driving ability, quality of life, and progression to overt HE.

Question 2

How should I grade HE in an intubated or sedated patient?

Accepted Answer

Sedation and intubation confound the West Haven assessment. Document the pre-sedation grade if available. If not, the grade cannot be reliably assessed. Consider using the Glasgow Coma Scale as an alternative measure of consciousness in ICU settings.

Question 3

Does lactulose or rifaximin treatment change the West Haven grade?

Accepted Answer

Yes — treatment response is assessed by improvement in the West Haven grade. A decrease of one or more grades indicates a therapeutic response. The guideline-recommended goal of lactulose therapy is to achieve 2–3 soft bowel movements per day and improvement in at least one West Haven grade.

Question 4

How do I differentiate hepatic encephalopathy from other causes of delirium?

Accepted Answer

This is a critical diagnostic step. Look for a supportive clinical context (cirrhosis with portosystemic shunting), elevated ammonia (though not diagnostic), characteristic EEG changes (triphasic waves), and exclusion of other causes of altered mental status — intracranial bleed (CT head), electrolyte disturbance, infection (especially SBP), alcohol withdrawal, and sedative medications.
