Question 1

Can the Wells DVT score be used in pregnancy?

Accepted Answer

The Wells score has not been specifically validated in pregnancy. Pregnancy DVT risk assessment requires adapted tools because leg swelling, oedema, and dyspnoea are common in normal pregnancy. D-dimer is also physiologically elevated in pregnancy, limiting its utility. Ultrasound is often the first-line investigation in suspected pregnancy-associated DVT.

Question 2

What D-dimer threshold should I use with the Wells score?

Accepted Answer

For patients aged ≤50 with Wells 'DVT unlikely', a standard D-dimer threshold (<500 ng/mL FEU) is used. For patients >50, an age-adjusted threshold (age × 0.1 mg/L for patients >50) improves specificity while maintaining sensitivity.

Question 3

Does the Wells score apply to upper extremity DVT?

Accepted Answer

The Wells DVT score was developed for lower extremity DVT. Upper extremity DVT has different risk factors (central venous catheters, pacing wires, thoracic outlet syndrome, cancer) and is better assessed using the Constans score or ultrasound directly.

Question 4

What is the role of the Wells score in suspected recurrent DVT?

Accepted Answer

The Wells score can be used in suspected recurrent DVT, but the clinical assessment is more challenging because residual ultrasonographic abnormalities (post-thrombotic changes) are common. A positive D-dimer and comparison with prior imaging are particularly important in this setting.
