Q: Does WHtR work for children and adolescents?

Yes. The same <0.5 threshold has been shown to predict cardiometabolic risk in children aged 6–16 years. Some age-specific centile charts exist for research applications.

Q: Is WHtR a better predictor than BMI?

Multiple meta-analyses indicate WHtR has a similar or slightly better discriminatory power for hypertension, diabetes, and dyslipidaemia compared with BMI and waist circumference. The main advantage is its simplicity and the universally applicable threshold.

Q: Can WHtR be used in pregnancy?

It is not validated in pregnancy. Pre-pregnancy WHtR may predict gestational diabetes risk, but the ratio changes significantly during gestation and should not be used for ongoing monitoring.

Question 1

Where should the waist circumference be measured for WHtR?

Accepted Answer

Measure midway between the lowest rib and the iliac crest (WHO protocol), at the end of a normal expiration. The tape should be snug but not compressing the skin.
