Q: What is the MCID for the TUG test?

The minimal clinically important difference for the TUG test is approximately 1–3 seconds depending on the population and baseline score. For patients with hip fracture, the MCID is about 2 seconds. A change of 2–3 seconds in the same direction on serial testing is likely clinically meaningful.

Question 1

Can the TUG test be used in patients who use a walking aid?

Accepted Answer

Yes — the patient should use their usual walking aid. The type of aid used should be documented as it affects the time. Patients using walkers typically complete the TUG more slowly. Re-testing with the same aid ensures consistency. Standardised footwear (no slippers) should be worn.

Question 2

What if the patient cannot safely perform the TUG test?

Accepted Answer

If the patient cannot rise from the chair, cannot stand safely, or refuses, document the reason. This inability is itself clinically significant — it indicates severe mobility impairment and high falls risk. Alternative mobility assessments (e.g., Functional Reach Test, De Morton Mobility Index) may be considered.

Question 3

Is there a ceiling effect with the TUG test?

Accepted Answer

Yes — the TUG test has a ceiling effect in high-functioning older adults. For community-dwelling active older adults, the TUG may be normal (<10 seconds) even when subtle mobility impairment exists. The Short Physical Performance Battery (SPPB) or gait speed alone may be more discriminating in this population.

Question 4

What is the MCID for the TUG test?

Accepted Answer

The minimal clinically important difference for the TUG test is approximately 1–3 seconds depending on the population and baseline score. For patients with hip fracture, the MCID is about 2 seconds. A change of 2–3 seconds in the same direction on serial testing is likely clinically meaningful.
