Question 1

Which method is more accurate — thick or thin film?

Accepted Answer

Thick films are approximately 10× more sensitive for detecting low-density parasitaemia (limit ~5–10 parasites/μL vs ~50–100/μL for thin films). Thin films are better for species identification and accurate percentage parasitaemia. Use both in parallel when possible.

Question 2

How do I distinguish gametocytes from asexual stages?

Accepted Answer

Gametocytes have a distinctive shape: P. falciparum gametocytes are crescent (banana) shaped; P. vivax and P. ovale gametocytes are round and fill the RBC. Only count asexual stages (rings, trophozoites, schizonts) for parasite density — gametocytes do not cause clinical illness and respond differently to antimalarials.

Question 3

Can RDTs replace microscopy for density measurement?

Accepted Answer

No. Rapid diagnostic tests (RDTs) provide qualitative or semi-quantitative (e.g., HRP2 band intensity) results only. They cannot determine species or density accurately. Microscopy is required for density quantification, severity assessment, and monitoring response.
