Clinical context
The INHOMESSS home visit assessment structures an in-home evaluation across nine domains — Immobility, Nutrition, Housing, Others (support network), Medication, Examination, Safety, Spirituality and Services. Each domain is rated OK, concern or critical, with space for findings and notes.
Use it for frail, multimorbid or homebound patients to uncover what clinics miss — fall hazards, polypharmacy, food insecurity, caregiver strain — and convert findings into prioritised environmental modifications, service linkage and realistic, context-aware plans.
