Question 1

Can the diagnostic rule rule out septic arthritis?

Accepted Answer

No — gout and septic arthritis can coexist. Any patient with acute monoarthritis and signs of infection should have urgent synovial fluid Gram stain, culture, and crystal analysis.

Question 2

Does the rule apply to chronic gout or polyarticular presentation?

Accepted Answer

The rule was designed for acute monoarthritis of ≤14 days duration in a single joint. It has not been validated for chronic, polyarticular, or tophaceous presentations.

Question 3

How reliable is serum urate during an acute flare?

Accepted Answer

Serum urate may be falsely normal during an acute attack due to inflammatory uricosuria. Repeat the test 2–4 weeks after the attack resolves if suspicion is high.

Question 4

When should synovial fluid analysis still be performed despite a high rule score?

Accepted Answer

If presentation is atypical, if the patient is immunosuppressed, or if septic arthritis is a concern. Also if the diagnosis will change management (e.g., initiation of long-term urate-lowering therapy).
