Question 1

How does GINA control relate to the ACT?

Accepted Answer

The GINA classification (well/partly/uncontrolled) uses four yes/no questions and is a categorical tool. The ACT provides a continuous 5–25 score. Both assess similar domains; the ACT has a validated MCID, making it more sensitive to change over time.

Question 2

What if a patient has no symptoms but uses their reliever regularly before exercise?

Accepted Answer

Exercise-induced bronchodilator use is not counted as 'reliever use more than twice a week' for the GINA control assessment. Separate exercise-induced bronchoconstriction management (SABA before exercise or a low-dose ICS-formoterol maintenance and reliever strategy) may be needed.

Question 3

Can I use the GINA control assessment in a patient just starting therapy?

Accepted Answer

Assess control 4 weeks after initiating a new treatment step. Earlier assessment captures transient improvement, not the sustained effect.
