Question 1

Can I use ECOG PS in non-cancer patients?

Accepted Answer

Yes — ECOG PS has been used in end-stage renal disease, COPD, heart failure, and other chronic conditions. However, its validation is strongest in oncology, where it was developed and where most prognostic data exist.

Question 2

How reliable is ECOG PS between different clinicians?

Accepted Answer

Inter-rater reliability is moderate (weighted Kappa 0.5–0.7). Agreement is highest at the extremes (PS 0 and PS 4) and lowest in the intermediate categories. When the score affects treatment decisions (e.g., clinical trial eligibility), consider using a multi-rater assessment or objective functional measurements (gait speed, grip strength, 6-minute walk) as adjuncts.

Question 3

Is ECOG PS the same as Karnofsky Performance Status?

Accepted Answer

They are highly correlated but use different scales. ECOG PS is simpler (6 levels vs. 11 for Karnofsky). Conversion: ECOG 0 = Karnofsky 100%, ECOG 1 = 80–90%, ECOG 2 = 60–70%, ECOG 3 = 40–50%, ECOG 4 = 10–30%. Both scales are accepted in clinical trials.

Question 4

Should I re-assess ECOG PS during treatment?

Accepted Answer

Yes — performance status often changes with treatment. Re-assess before each treatment cycle. A decline by ≥ 2 points during therapy may signal treatment toxicity or disease progression and should prompt re-evaluation of the treatment plan.
