Q: Can CRC differentiate between iron-deficiency anaemia and thalassaemia trait?

CRC does not directly differentiate these. Both are hypoproliferative (low CRC). The Mentzer Index (MCV/RBC) and RBC count are more useful: thalassaemia trait typically has a very high RBC count for the degree of microcytosis, whereas iron deficiency has a low-to-normal RBC count.

Q: What is a normal corrected reticulocyte count?

In the absence of anaemia, the CRC is approximately 0.5–1.5%. In the presence of anaemia, a CRC 2.5% in an anaemic patient suggests appropriate compensation.

Question 1

What is the difference between corrected reticulocyte count and reticulocyte production index?

Accepted Answer

CRC adjusts for the degree of anaemia only. RPI adds a second correction factor for the premature release of reticulocytes (shift cells) in severe anaemia. RPI is more accurate in moderate-to-severe anaemia (Hct <30).

Question 2

Can CRC differentiate between iron-deficiency anaemia and thalassaemia trait?

Accepted Answer

CRC does not directly differentiate these. Both are hypoproliferative (low CRC). The Mentzer Index (MCV/RBC) and RBC count are more useful: thalassaemia trait typically has a very high RBC count for the degree of microcytosis, whereas iron deficiency has a low-to-normal RBC count.

Question 3

What is a normal corrected reticulocyte count?

Accepted Answer

In the absence of anaemia, the CRC is approximately 0.5–1.5%. In the presence of anaemia, a CRC <2% suggests inadequate bone marrow response. A CRC >2.5% in an anaemic patient suggests appropriate compensation.

Question 4

How soon after starting iron therapy does the CRC increase?

Accepted Answer

The reticulocyte count begins to rise within 3–5 days of initiating oral iron therapy, peaking at 7–10 days. A rising CRC confirms an appropriate bone marrow response and serves as an early indicator of effective treatment.
