Question 1

Can CAM-ICU be used in non-ventilated patients?

Accepted Answer

Yes — CAM-ICU is validated for both ventilated and non-ventilated ICU patients. For non-ventilated patients, a simple conversation can assess attention and thought organisation.

Question 2

How do I assess CAM-ICU in a heavily sedated patient?

Accepted Answer

If the patient is deeply sedated (RASS −4 to −5), they cannot be assessed for features 2–4. Document RASS and re-assess once sedation is lightened to RASS ≥ −3.

Question 3

What is the sensitivity and specificity of CAM-ICU?

Accepted Answer

Original validation showed sensitivity 93–100% and specificity 89–100%. Meta-analyses report pooled sensitivity of 76–84% and specificity of 96%.

Question 4

How does CAM-ICU differ from the standard CAM?

Accepted Answer

CAM-ICU is adapted for the ICU — it does not require verbal responses and uses a picture-based attention screen for intubated patients. The standard CAM requires full verbal communication.
