Q: Do I need multiple BP readings before staging?

Yes. Stage based on the average of ≥2 readings on ≥2 occasions unless the reading is ≥180/≥120 (hypertensive crisis) or the patient has evidence of acute hypertension-mediated organ damage.

Q: How does the tool handle systolic and diastolic staging discordance?

By convention, the higher of the two stages is used. For example, BP 150/85 is Stage 2 (ACC/AHA) or Grade 2 systolic / Grade 1 diastolic — the systolic stage determines the overall category.

Question 1

Which staging system should I use in my practice?

Accepted Answer

Use whichever system your national or local guidelines endorse. The ACC/AHA system is common in the Americas and parts of Asia; ESC/ESH is widely used in Europe, the Middle East, and Africa. The threshold for initiating pharmacotherapy differs slightly, but both target <130/80 mmHg for most treated patients.
