Question 1

Does the BMS apply to neonates younger than 29 days?

Accepted Answer

No — the BMS was derived and validated in children aged 29 days to 19 years. Neonates have different pathogens and immune responses; separate decision rules (e.g., the Rochester criteria) should be used in the first month of life.

Question 2

Can a BMS of 0 override clinical concern?

Accepted Answer

No. The BMS is a risk-stratification aid, not a substitute for clinical assessment. If the child appears toxic, has petechiae/purpura, or there is high clinical suspicion despite a score of 0, proceed with empiric treatment.

Question 3

Does the BMS apply to immunocompromised children?

Accepted Answer

The BMS has not been validated in immunocompromised populations. Children with HIV, asplenia, complement deficiency, or on immunosuppressive therapy should not be managed by the BMS alone.

Question 4

How does prior antibiotic use affect the score?

Accepted Answer

Prior oral or IV antibiotics can partially treat meningitis, lowering CSF cell counts and protein levels. The BMS may underestimate risk in this setting. Use clinical judgement and consider alternative diagnostic approaches (e.g., PCR).
