Question 1

How does AIMS65 compare to the Glasgow-Blatchford score (GBS)?

Accepted Answer

GBS is more sensitive for predicting need for intervention (transfusion, endoscopy, surgery) and can identify patients with a score of 0 who are safe for outpatient management. AIMS65 better predicts mortality and is simpler to calculate. Many guidelines suggest using GBS for pre-endoscopy triage and AIMS65 as an adjunct for mortality risk.

Question 2

Does AIMS65 work in variceal bleeding?

Accepted Answer

AIMS65 was developed and validated in mixed UGIB populations including variceal and non-variceal causes. Its performance in purely variceal bleeding is less well studied; use in conjunction with disease-specific scores such as Child-Pugh or MELD-Na when varices are the suspected source.

Question 3

What if the patient is on anticoagulation?

Accepted Answer

INR is a component of AIMS65, so anticoagulation will increase the score. This is appropriate, as anticoagulated patients have higher mortality in UGIB. However, do not delay resuscitation or endoscopy while calculating the score.

Question 4

Can AIMS65 be used in patients with chronic liver disease and baseline abnormal INR/albumin?

Accepted Answer

Yes, but interpret with caution. Patients with cirrhosis often have low albumin and elevated INR at baseline, which may inflate the score independent of the acute bleed. Use alongside clinical judgement and disease-specific scores like Child-Pugh or MELD-Na in this population.
