Question 1

Should I adjust the A-a gradient for altitude?

Accepted Answer

Yes — at high altitude, the lower Patm reduces the PAO₂. The tool accounts for this if you provide a non-standard Patm. The normal gradient reference ranges do not change with altitude, but the absolute value of PAO₂ and therefore PaO₂ is lower.

Question 2

What FiO₂ should I use for a patient on nasal cannula?

Accepted Answer

As a rough estimate: 1 L/min ≈ 24%, 2 L/min ≈ 28%, 3 L/min ≈ 32%, 4 L/min ≈ 36%, 5 L/min ≈ 40%. Above 6 L/min, use a non-rebreather mask (≈60–80%) or calculate precisely from the ventilator settings. The A-a gradient is most reliable on 100% FiO₂ when shunt fraction is suspected.

Question 3

Can the A-a gradient be normal in pulmonary embolism?

Accepted Answer

Yes — up to 15–20% of patients with acute pulmonary embolism have a normal A-a gradient on room air, particularly those with small, distal emboli and no prior cardiopulmonary disease. A normal gradient does not rule out PE.

Question 4

What is the effect of hyperventilation on the gradient?

Accepted Answer

Hyperventilation lowers PaCO₂, which increases PAO₂ via the alveolar gas equation, thereby widening the A-a gradient at a given PaO₂. Interpret the gradient in the context of the PaCO₂.
